1. Payment Authorization

The client acknowledges and confirms that they are authorizing payment for services rendered by 311 Aesthetics Co. The payment covers all outstanding balances related to the services agreed upon between the provider and the client.

Due to the nature of the work required to prepare and deliver these services, no refunds will be issued after payment has been received.

A non-refundable & non-transferable deposit is required to secure a booking for all services.
The deposit will be deducted from the total cost of the service.
The remaining balance must be paid in full at least 7 days before the event date, unless otherwise agreed upon in writing.

Wedding Officiation: If the client is 15 minutes late or more, a $50+ late fee may apply.
If the client fails to appear at the scheduled time without prior notice, 100% of the service fee will be charged.
Decor Setup & Planning: If the venue is not accessible at the agreed-upon time, additional fees may apply for delayed setup or extended service time.

Clients must provide at least 72 hours’ notice for rescheduling event services.
Only one rescheduling is allowed.
Cancellations within 72 hours of the event will result in a charge of 50% of the total service cost.
Emergency reschedules may be considered at the discretion of 311 Aesthetics Co.

6. Officiant’s Responsibilities

The officiant agrees to:
Officiate the wedding ceremony according to the agreed-upon script or format.
Arrive at the venue at least 30 minutes before the scheduled start time.
Coordinate with the client and other vendors as needed.

The client agrees to:
Provide all necessary documents required for the ceremony.
Notify the officiant of any changes at least 24 hours before the wedding.
Ensure the officiant has access to the venue and any necessary equipment.
Confirm that the ceremony is conducted within Cook County, Illinois, USA (or any pre-approved location).

Both parties agree to comply with all legal requirements and obtain any necessary permits or licenses for the wedding ceremony.

9. Scope of Services
9. Scope of Services
The decor and planning services may include but are not limited to:
Decor Setup: Backdrops, tablescapes, floral arrangements, centerpieces, and other decorative elements.
Event Coordination: Overseeing the decor execution according to the agreed-upon plan.
Cleanup & Breakdown: Removal of decorations.

Access to Venue: The client must ensure 311 Aesthetics Co. has access to the venue at the agreed time for setup and breakdown.
Extended Setup/Breakdown: If additional time is required due to venue restrictions or delays caused by the client or third parties, extra fees may apply.

If the client provides decorations or materials, 311 Aesthetics Co. is not responsible for any loss, damage, or incorrect setup due to missing or faulty items.
If any decor items are damaged, misplaced, or stolen during the event, the client is responsible for covering replacement costs.

311 Aesthetics Co. reserves the right to refuse service or terminate a client relationship at any time for reasons including but not limited to:
Unreasonable client demands outside the agreed-upon contract.
Unforeseen circumstances that prevent service fulfillment.
Any behavior that is deemed disrespectful or inappropriate toward staff.
13. Liability & Indemnification

311 Aesthetics Co. is not liable for any accidents, injuries, property damage, or unexpected circumstances that may arise during the event.
The client agrees to indemnify and hold 311 Aesthetics Co. harmless from any claims, legal actions, or disputes resulting from the event.
14. Governing Law

This contract shall be governed by the laws of Cook County, Illinois, USA.
15. Media Release & Promotional Use

By hiring 311 Aesthetics Co., the client agrees that their name, image, and likeness may be used in promotional materials, including photographs, videos, and digital content.
311 Aesthetics Co. may distribute this content on its website, social media, and marketing materials.
No compensation will be provided for the use of promotional images.
Clients may withdraw consent in writing, and 311 Aesthetics Co. will make reasonable efforts to remove their likeness from future materials.

By booking services with 311 Aesthetics Co., the client acknowledges and agrees to the terms outlined in this document.
